If in the basal group, sulfonoureas had been elinated and TZDs used with metformin instead, and Januvia or Onglyza used instead of postprandial insulin the control would have been much better and the incidence of hypoglycemia markedly reduced. In this day and age there is little justification for the use of biphasic insulins in type II diabetes.
Reason to use biphasic insulins

I agree that medically there is little indication for biphasic insulins. But in the real world, finances sometimes are more important than the actual medicine. And the biphasics are generally cheaper and you can get away with only two shots a day.
